When Grief Gets Stuck: Complicated Grief

Most grief softens over time. Complicated grief doesn't — it stays raw, consuming, and unrelenting.

What Is Complicated Grief?

Complicated grief (also called prolonged grief disorder) occurs when the natural grieving process gets 'stuck' and intense grief continues without the gradual softening that usually occurs.

It's not about timelines — there's no deadline for grief. But complicated grief is distinguished by its intensity remaining unchanged months or years later.

Approximately 10-15% of bereaved people develop complicated grief. It's not a weakness — it's a condition that responds to specific treatment.

Signs of Complicated Grief

Intense longing and yearning that doesn't decrease over time.

Difficulty accepting the death — it still feels unreal months or years later.

Bitterness or anger about the death that dominates your emotional life.

Feeling that life is meaningless or purposeless without the person.

Difficulty engaging with memories — either avoiding them entirely or being consumed by them.

Social withdrawal and isolation.

Inability to pursue interests, goals, or relationships.

Try This Now

Honest assessment: Has the intensity of your grief changed at all since the death? If it hasn't — if the pain is as raw now as it was in the first weeks — this may be complicated grief, and specific treatment can help.

One step toward life: Complicated grief can make you feel that re-engaging with life is a betrayal. But the person you lost would want you to live. Do one thing today that is 'life-facing' — see a friend, go somewhere new, try something.

Write about the relationship: Write about the person: the good times, the difficult times, the things left unsaid. Getting the full picture on paper can help your brain begin processing.

When to Seek Professional Help

If grief is unchanged in intensity after 6+ months.

If you're unable to function in daily life because of grief.

If you've had thoughts of wanting to die to be with the person.

Complicated grief treatment (CGT) is a specific, evidence-based therapy that helps process grief that's become stuck. It works.
